TERMS
AND CONDITIONS OF PLACEMENT/ ADMISSION PROCEDURES
A.
Upon placement or decision to reside, the degree of care necessary will
be fully discussed with the Service User, Family members, Carer or Social
Worker, to agree and decide what the proposed Service user is able to
do for herself/himself and what assistance will be required from the staff
at Brookwood Manor Residential Care Home. All care needs agreed at the time
of placement will be included within the fees payable. Note:- Should care
needs significantly increase it may be necessary to meet with all parties

To ensure that a complete,
thorough and fair assessment is made it maybe necessary to contact the
following professional personnel:
Occupational Therapist
Physiotherapist
Consultant Geriatrician
Reports will be available
to Brookwood Manor by these relevant personnel.
B.
The first four weeks after a Service User enters The
Brookwood Manor are on a long term basis (permanent), will be mutually recognised as
a trial period. This will allow the Service User to have a change of mind
and see how the new Service User is fitting into the surroundings. Should
alternative arrangements become necessary because the placement proves
unsuitable, Brookwood Manor will assist, in conjunction with the Service Users
Carer/Social Worker to pursue alternative arrangements. We regret that
unless the room to be vacated can be filled immediately, a notice period
of 4 weeks will be required.
C.
A decision to reside, in many cases will be traumatic and a difficult
time. We will, therefore, be willing to visit the prospective Service
user before-hand in their own home or hospital for an informal discussion,
to allay concerns, and offer any opportunity to visit Brookwood Manor to meet
both Staff and other Service users.
D.
Brookwood Manor is a home for the Elderly with Dementia and has catergory EMI and caters for an age range of 65 upwards.
E.
Brookwood Manor will be pleased to accept both Male and Female Service users.
